phal 'glad dawn' double flower

i felt it was taking a long time for a certain flower to open up, and when it finally did, it was a double flower:

how often does this happen

Wow, that's neat. Do you know what causes that? Or is it just a defect? Are all the flowers on that plant like that or is this just a random occurance?

I have no idea what actually causes it, though I believe it to be just a random occurance. There are 2 other buds on the plant, but judging by the size of the flowers the plant had last time, they have a while to go before opening.
