From facebook click on your settings icon (sprocket looking thing upper right corner by the lock). From there, on the left near the bottom click on "Apps". Click the X on the far right hand side of the "TerraForums.com" app. Confirm to remove TerraForums.com. Done! There shouldn't be anything to do on TF to finish the removal.
