Pitcher rot

I have some nice ventricosas, and on each of them, a pitcher has begun to rot and fall off. Does this just happen? They have been thriving for almost a year now, and no conditions have changed... Anything to look out for? And what might have caused it?

Perhaps dust or dirt if they are outside. Something large like roaches or waterbugs.

How old are the pitchers? Is there anything in them?
